where can i find donated furniture for the homeless trasitioning into their own homes/ apts?

Find your local Freecycle group, every city has one and post your "wanted" needs on there. There is also Goodwill and those type places that have affordable used furniture. In the past there were vouchers that could be obtained from the government to help the poor get furniture. I don't know if these vouchers exist anymore considering that the government only wants to help the rich anymore and they seldom need help with such things.
