Twice I believe to transport to Thor's people and/ or the gate builders. It is like a control device, cannot be accessed from earth or the gould worlds.

That we actually see? Twice: "The Fifth Race" and when they wormhole-jumped to the Supergate in "Pegasus Project". Now, when you combine Atlantis with SG-1, you get a few more times... but since I don't watch Atlantis, I can't quite give you a count (though once for sure in the pilot episode).

I think 3 times, once to the Asgard's homeworld and twice to Atlantis.

i thinks it a few more times now since they have been going back and fourth from atlantis how many not sur at least 6
