Disodium = 2 sodiums
ammonium = NH4

therefore the form is Na2 (NH4)(PO4)

Na2(NH4)PO4, the reason is because there is +1 charge each for Na and NH4, and there is -3 charge for PO4, so when they want to become chemically balance, +2 from 2 Na and +1 from 1 NH4 will combine with -3 from 1 PO4 to become neutral in their charge, and thus become chemically balance.
