The form that I'm trying to print is at http://www.ctc.ca.gov/credentials/leaflets/41-LS.pdf.

I have tried saving it into my computer and printing, but cannot do so. Whenever I try to print, the Print to File box appears and asks for an output file name. I have tried entering various names for the file, but no printing takes place. It's not my printer because it can print Word documents and pictures without any problems. Please help.

Sounds odd. if you open the PDF in Acrobat reader, goto FILE > PRINT. Check that the printer selected is your printer. If you have Microsoft Document imaging or a PDF printer installed then you ussaully see a box that asks you to enter and output file name.

if you recently installed a PDF printer it may have made itself the default printer and that would explain why you have that problem.
