You can call 1-800-829-4477 and put in TeleTax topic 154. Just follow the instructions they give you and then punch in 154. That will tell you what to do.

I would call like H&R block just for advice.
Or call the IRS and ask them.
Employers had until January 31st to get them to you.

you dont really need it all you need is your last paycheck

but try contacting your companies human resources department
