www.acop.com is a great survey site--it's the first one I signed up with (about 5 years ago) and I've had no trouble with them. They've never sent any spam or junk mail--just surveys. They usually pay about $5-15 per survey, which is on the high end for surveys. They pay by check.

Just go to their website and you can sign up for free. They will probably have you fill in some profile questions, plus give your name and adddress where they can send your payments.
