Need not pull back the fore skin. It will automatical slide down when you insert. Roll down the condom till the condom ends. It may end at the middle or at the bottom depending upon the length of your erect penis.

in normal cases always the indian condems will be long enough. if you have any problem in that part of skin like stretching it back - go for a minor surgery, circumsition

Yes, you need to roll it (foreskin) back first.
Wear the condome as much as your length. If the condome you have is smaller than your erect tool, reward yourself and get another larger one :)
