my brother, he has a mental problem, also he is slow, and he is lazy, he takes a shower however he still smells like he didn,t wipe his ***, he didn,t wash his ***, what can l say to him ?

When you say "he has a mental problem" do you mean that's your opinion of him, or is he what's called "special needs?" If it's the former, well, he's a free moral agent and there's not too much you can do outside of talking to him about the odor and hoping he'll correct it. If he's the latter, there's also little you can do. I've worked with special-needs kids as a teacher's aide and most of them smelled, partly because they couldn't control their bowel movements or bladder. I'm not sure--I didn't think to ask--but I have the feeling that, if the smell doesn't arise from the bladder and bowel-movement problem, it's a byproduct of their condition.
If he's overweight it's likely he has hemorrhoids, which would explain the fecal smell. That's a diet-related problem; he would need to cut out, or severely cut down on, processed foods: all sweets, anything from fast-food restaurants, soda, chips, etc. Drink more water/liquids; exercise; eat more fiber-rich foods like fruits and veggies (you can usually eat anything raw that's normally eaten cooked, like potatoes, broccoli, lettuce, and so forth). Too much alcohol and coffee are also not good.

put a reminder on the mirror saying don't forget to wipe and don't forget to wash your hands. tell him that it is a reminder for the both of you. every time he sees it. maybe he will wipe and wash.

Perhaps he is suffering from depression. See if he will agree to be evaluated by a mental-health professional.
