As for as your birth certificate, you will need to contact the county registrar of the county which you were born in and request a copy.

I do believe that you can contact the social security administration and request a reprint for the card. Please refer to the web address below:

"Limits to replacement cards

You can replace your Social Security card if it is lost or stolen. You are limited to three replacement cards in a year and 10 during your lifetime. Legal name changes and changes in noncitizen status that require card updates may not count toward these limits. Also, you may not be affected by these limits if you can prove you need the card to prevent a significant hardship.

Go to or call your local Department Of Health-Division of Vital Statistics, you can get a Certificate of Live Birth there or they can tell you where to get one-you will most likely have to pay to get an official copy. For a Social Security Card-go to the Social Security Office and they can re-issue you another card-will take up to/around 15 days to receive it.

For the birth certificate, you would have to write to the State in which you were born in.. usually the Department of Health (Vital Records section). The SS Card.. you'd have to call the SS Administration and find out what kind of information they'd need. I think a birth certificate just might be one form of identification they'd want.

The birth certificate is easy; contact the registry of vital statistics in the city or county where you were born. (Expect to have to pay a fee.) As for the card, contact the Social Security Administration. (Actually, I can think of nothing whatever for which you would need the card, as long as you know what the number is.) But contact the credit bureaus, and put a fraud alert on your files -- this will save your butt if the documents wound up in the wrong hands.

I haven't seen my ss card for decades. As long as you know the number, you are ok. You don't really want to carry that around, if you lose it, people can steal it.
