i have only seen the 1st 8 episodes as just got season 1 dvd,so if its summat that they explain later i not seen yet,lol

It's really from the first episode, and just a running joke. JD is I believe the last one in to the hospital the first day and he sort of gets caught in the door when coming in and for some reason the janitor just decides to pick on him and continues throughtout the series. If you watch, as the second season starts and the first year students are finally "graduating" the new students are coming in and the last one in does basically the same thing that JD did when he first came in and the janitor starts picking on him for no reason at all. But as JD is the STAR the janitor continues to hassle him continually throughout the show. As I said, a running joke, just something to keep people laughing. Hope this helps somewhat. The show holds together, even if you don't watch it all in order. I watched the first season and then started watching it out of order and still found it funny.

In the pilot, the Janitor is fixing a broken door and JD suggests there might be a penny stuck in it. It turns out there was a penny stuck in it and the Janitor assumes JD put it there and holds it against him.
JD is constantly unlucky in that he accidentally does things to cause problems for the Janitor and so their relationship continues in this manner.

In the first episode...
The janitor was working on an automatic door and said it kept breaking. Jd said jokingly that "maybe there was a penny in it" the janitor later finds a penny and holds a grudge (plus I think he just enjoys it)

I think he just sees Dorian as an easy mark. I think the relationship between those two is hilarious.

duh! JD has been mean to the janitor since the jirst day he got there. Its all explained in the musical episode. duh!!
