That seems a little petty to me, changing your child's name just because you are not longer with its mother. You could change your child's last name, but given the situation you are in, and the age of your child, you would have to go through a court system, instead of the normal file request of forms and paying.
But I'd be more thinking about how your child will grow up to think of you both if you do things now just because you don't get along.

Oregon has some funny laws about names of children - I believe the name can be changed within the first year. But if the child was born in Utah, the laws there would apply. Contact the county where the child was born quickly and find out what's legal there. Then discuss it with the child's mother.
If your name isn't on the birth certificate, I would make sure that that happens first. Then look at what's best for the child. Too many of the teens today have no self worth - in part because they feel that if their father didn't care enough to even be listed on their birth certificate, what are they worth? You child may grow up to have the same problem - if you as parents can't even decide what his/her name will be, how can they ever expect you to get along enough to care about them, not yourselves first. Maybe a compromise is to use a hyphenated name - showing you both care about the child.
