I'm looking for the most sick, twisted, hard-to-watch films. Don't say "Nightmare on Elm Street" or anything mainstream or cheesy like that. Banned underground movies are a plus. Thanks a lot!!

Ok this one is really sick. I ACTUALLY puked watching this movie, In the movie they actually kill a real turtle.... And you see it... It's called Cannibal Holocaust

August Underground’s Mordum ....hands down...but onlyby a hair...
if you have a barf bag handy, and a warm towel to wipe your mouth...and a liter ofcold ginger ale for a stomach that will get more upset...
then try
1) Irreversible
2) Cutting Moments
3) Guinea Pig: Flower of Flesh and Blood
4) Murder: Set pieces
5) Men Behind The Sun ...
the last being based on a true story of atrocities committed by japanese soldiers on pow's during WWII.
Normally I give links where these can be found...think I'll pass this time.
I warn the casual observer to bypass these flicks...once you have these images in your mind...you may wish you hadn't viewed them in the first place. You've been warned.

Im sure it was before your time but I guarantee you it was "the wierd world of LSD" in about 1967. I somehow saw it at the drive in (again before your time). Its fits what you are looking for.
Check the description from the web. I cant believe I got a hit on the web search:
"This hilarious exploitation drama reveals the secret lives of LSD addicts with preposterous vignettes a la "Reefer Madness." "

"I’m going to try and offer up a description of what you can look forward to during the duration of this movie. There’s no plot or continuity, so bear with me. After a shot of the cosmos, a pre-credit sequence unfolds. An LSD indulged young man is flapping his arms on a couch. A deadpan narrator informs us that this guy believes he’s flying on the wings of a large bird. Superimposed over the man’s face is a cheap cartoon drawing of a chicken. Attention: the bar has been set. From here on out you can expect several variations on people’s reactions to LSD, usually based on their innermost anxieties and fears. Sometimes a couple of strange drawings and transitions appear between vignettes. Highlights here include a woman frolicking with kittens, another woman fantasizing in a mannequin factory, a large middle-aged man gorging himself on a gigantic spread of food (really just a ham sandwich, but you know how that sneaky LSD works), a restaurant table strip tease with scissors, a woman performing steamy, tiger-themed interpretive dance on a jungle set, and some drag racing stock footage. I also forgot the chocolate syrup gore, rubber monster mask spooks, and the guy who eats dirt. Soon enough, “The End” shows up onscreen for a second, before we’re left with an abrupt cut to black."
This was the sickest hour I have ever seen on film and perhaps the biggest waste of an hour of my life. It must be to have stayed with me since 1967.

First thing that came to mind was "Pink Flamingos".
Excellent if you want twisted, nasty movies. It's nasty. It's a John Waters gem from, i believe, the 70's, so they get away with a lot.
Very disturbing. Excellent choice.
Second thing I could think of is "Faces of Death". It's just wrong...but deliciously wrong!

there is a movie called "Strangeland" with Dee Snider from Twisted Sister...i wouldn't say it was the most sick movie...but it was interesting
