It means...ROCK ON!!!! or two outs if you are on a baseball field.

If you'll notice, they're probably putting their thumb against the two remaining fingers. I do believe it is a satanist symbol signifying the goat.

In Italy that symbolize horns; currently, the most common meaning is "you're wife (husband) is betraying you". Not in such a soft way :) Usually used along with an exclamation: "Cornuto!" which means "You bear horns in front of your head" ("Horned!") .
Originally, it was likable supposed to be a gesture to ward off bad luck, representing devil's horn.
