Have you ever noticed that when you see a man the picture is
clear, but when you see a woman the picture is slightly foggy.
I thought they only did that in the old movies, but I noticed that
they still continue to do that. It's subtle though interesting, why?

are you talking about the picture as a whole or the background? picture as a whole they use filters to soften the entire image as stated to soften skin imperfections, blemishes, etc. they also use warming filters to enhance skin tone...depends on the shot and mood needed. if you're talking about the background that's a depth of field shot. where the background is out of focus, blurred, making everything around the actor/actress soft thereby focusing your attention on the actor/actress.

They add the soft focus for a "romantic" effect, and also to help hide imperfections in complexion or makeup that may be revealed by a crisp camera shot. Vanity goes a long way in Hollyweird.

I've only noticed this when the women are old.

Maybe so the women looks more sensual. Especially if it is a chick flick.

If you are a man.. its because all your blood goes to some other part in your body than the brain.. that's why it looks soggy...

yeah it's a romantic dreamy effect, that and when they do that in the soaps they're hiding the actress' neck from looking wrinklely
