Make sure all the grooves is lined up and the new alternator pulley and has the same amount of tracks as the rest of the pulleys and check and make sure the new alternator is in line with the rest of the pulleys do this by sighting the pulleys of the alternator and see if it aligns with the rest of the pulleys not forward or back any at all but exactly straight with the other pulleys some time there are shims or washers that can go on either side of the mounting bolts and installed on the wrong side can get the alternator out of alignment good luck

I did this once, on my wife's 1992 4.3 liter Safari.
I replaced the front bolt holding the alternator in backwards, because it was easier to put the nut on. It destroyed the belt. I learned not to do that. Good luck.

Use a straight edge, or visualy check to see if the outer edge of the Alternator pulley is perpendicular to the other pulleys. (spaced right)
You may have neglected to put a spacer between the pulley, and the alternator shaft bearing.

The evidence is that the alternator is mismounted in some way -- either tilted, or offset. The belt tension may be in error also. The exact nature of failure of the belt may give a clue.
