I want to make an altered book for a wedding gift and every book I've found on the subject has less than clear instructions. Could anyone suggest a comprehensive book on the subject?

You don't really need a book. Use the internet. There are two books I haven't read called Altered Books 101 and 202(those are the approximate names. can be found at Michaels). There are just so many ways to do things and things to do to a book. I got one from my library by Terry Taylor but I wouldn't consider it comprehensive. He has been altering things for years. I've used several of his techniques. You can also search the internet. I found one website that has lots of info about starting the book.
In my experience (I've completed three books and am working on two others), It is imperative to find a book with a good binding. Don't use an old book. I'm about to experiment with a textbook. Board books are good because you don't have to rip or glue pages. Reader's Digest condensed books are a dime a dozen and the spines are decent.
Start by taking pages out. If you can find the center of the score (books are often bound in groups of pages called a score. if you get to the center of the score, you won't lose as many pages than if you ripped them out elsewhere in the score) Glue pages together to make them stronger. Then start painting or gluing other paper over the words. Acrylic paint works fine. You can put wax paper in between damp pages so they won't stick together. You should put wax paper in front and back of the pages you are working on so they don't get wet. Regular white (pvc) glue is fine for gluing pages. Don't get the washable school kind as it is way too watery (voice of experience).
Look up Altered Book online for lots of ideas.

I am not sure that there IS a comprehensive book on making an altered book. There are so many different ways to do this. There is a website called blankboard books (I think) you could purchase one there or go to a dollar store and buy a child's board book to use. Or order one of many kits out there. My suggestion is to find out how to alter things and use those tecniques. You will find varying opinions on how to do so. Or maybe find a local scrapbook instructor or store with instructors to teach you how! If you would like more information from me I would be happy to help you.

Are you sure the people would want an altered book? I just don't get the point of those things.
