I live in the Cleveland area and haven't seen one at any of the local stores. I've been on the lookout for a Sega CD (and an old 3DO unit too) myself for quite some time, with no luck. I have a buddy in Cincinnati too that works at Gamestop, and he hasn't seen any.
Best bet is Ebay. Although, if you're just looking for one game, you might want to consider downloading an emulator. My brother modded his X-Box to play Sega, Sega CD, and 32x games, and it works great. Not sure if its entirely legal (we owned em all at one time, but they are long gone), but it works.
You can also play them on your PC if you dont want to go through the trouble of modding another console system.

If Ebay doesn't have one you can get emulators on the internet that play games, and as you only want one game try geting one. Search for 'Sega CD Emulator' on yahoo.

Try ebay. Or a used video game store other than Gamestop or EB Games. Usually a small independent store will have more of a selection of older titles and systems.
