Simply stated, "Can you imagine youself wanting to have sex with that person?" That is sex appeal. And the x factor is that something extra that you have that the other person does not.

I very much doubt that every reality show is based on sex appeal: Fear Factor, for instance. And yes, people know the meaning of both... obviously or they wouldn't exist.

Sex appeal means appealing to sex / tempting to have sex / having seductive personality (inviting to have sex), sensuous figure,who arouse sexual feelings.
X factor is finding hidden beauty in an un-appealing personality and enhancing small factor to cover remaining deformities.

X-factor is interesting. I would say its an attractive factor that defies populist wisdom or currency. For example, you could be conventionally ugly and short and a little overweight but a person / persons may find you attractive because there is something a little special about you. Something not tangible or accountable that makes you attractive.
