Any animal that lives in close proximity to its own droppings is at risk for carrying Salmonella- but this does not mean that every individual does.
The odds that an animal living in a decently maintained dry tank has Salmonella is moderately low- but you cannot tell if the animal is carrying by looking at it. Therefore we treat ALL small animals as if they may be infected.

Yes
I mix up some povidone iodine in water and let them swim in it for 5-10 minutes then give my water dragons a bath in dechlorinated water as the iodine is drying to the skin, but will kill most of the salmonella bacteria.

Yes.The easiest time to catch it is when their shedding. If you wash your hands after handling them with an anti-bacterial soap, you'll be fine. Also many turtles also carry salmonella such as red ear sliders.

No. They can get it though, just like humans can. Just wash your hands before and after handling a lizard.

yes they all carry the disease. last week I was injecting my iguana and water dragon with some antibiotics prescribed by the vet, after injecting the medicine the needle accidentally went into my arm, I had to go to get a shot to be sure I would not get the disease.
