It was originally marketed as a diary of an anonymous teenage girl edited by Beatrice Sparks. Although no one but Sparks knows for sure, it has been widely speculated that Sparks contributed much more than she orignally let on. There has been research through the newspapers at the time, and no deaths were reported that fit that description at the time, and other inconsistencies.

Still, it is a powerful book every teenager should read. Even if it is an compilation of different stories edited together for maximum impact, it is still compelling, iconic, and and a powerful message.
