Feeling Good -subtitled A New Mood Thearapy it is an old book but I believe it is kept current by reprints and is still popular by demand. It's chocked full of self-evaluations, tests and scales. Answering the questions on paper will preserve the text of the book for future evaluation or for sharing with others. The book also makes an awesome gift for troubled friends/family and is in paperback form. (It Should be super easy to find on Amazon or in any search engine.
