Say something to her about it. Obviously you can't leave the kids in that state....it ain't their fault so why should they suffer.....not that I think you were considering that course of action anyway...

report her annomymously to child protective services. That is a sign of neglect and could be just the tip of bad living conditions.

I went through this with my neice's mother. The baby was ALWAYS dirty and had sever diaper rashes. Come to find out her mother's house was complete squaller! Not even fit rats I swear. Filth beyond comprehension and highly unsafe conditions for a child.

At least cps can check it out and make sure those babies are ok. That is the important thing.

you should advise them that they need to fix the problem that you are having with thier childrens hygine. its obvious that you care about the kids or else you wouldnt be telling us! and those children are so close together (age-wise) the mother may need the help that you are giving her trust me she appreciates it or she wouldnt keep coming back to you. try to get into her head and see what the problem is it may be deeper than you think. just keep helping out as much as you can itll pay off in the long run!

Say, "Susy, do you think you could change them before you get here? My lower back problem is acting up, and the less I lean over and change diapers the better it is for me."
