Do you mean the small douche bag? It usually has a white wand that screw on at the opening, once your fill it with water and what ever you want to add. Vinegar or massengill.
If that is not it, then it can also be used for enimas. Filling of warm water and with the wand also, inserting it in the rectum and slowy squeezing the bag so the water goes up the intestine.
That way if someone is constipated, there feces would release out of the body with ease than a squeeze.

Here in England rubber hot-water-bottles are still widely used and are certainly not considered by most people to be old fashioned. They are wonderful for period pains, or aches and pains generally. Most of all they are just lovely for comfort and warmth and to cuddle in bed at night, especially cold nights. As for the alternative suggested uses (enemas and douches) - I've never heard of this before. Must be something exclusively American, and is probably why the questioner refers to them being “mysteriously hidden”.

Its a hot water bottle, sort of like a heating pad. If you have something that requires heat for comfort, you fill the thing up with hot water and apply it to the area like a heating pad. Its old fashioned, but very effective.
