Your just Fill the Form 49-A and fill the Form with Black ink pen attached your address proof & ID Proof one Colour Photo than you apply any NSDL Centre or UTI PAN ok form dowload website is

You can get your PAN number by applying on a form which is available in any income tax office.Alternately ,you can seek the help of any registered chartered accountant who files IT returns for his clients.
