The previous answers are correct but I would add one more necessity to the directions. You need to take the grout out that surrounds the broken tile. Otherwise when you hit it, the grout will hold the adjacent tiles to it and they have a good chance of breaking too.
Get a grout saw (carbide blade with handle) for about two bucks form Home Depot. Scrape out the grout on all of the sides of the defective tile. Then proceed with the smashing and replacing that were well described in the other answers.

Lightly tap the tile until it is broken enough to remove. Scrape or chisel the grout from the floor making sure to clean the edges next to the pieces left in place. Mix the new grout and reset the new tile in place making sure it is flush and level with the other tiles. Clean joints and let sit. Then after it has dried, mix the joint grout and apply. Be careful to get the same joint grout as the rest of the room.

hammer screw driver or chizzel and slowly with protective eye ware and gloves and any other safety so you dont get cut, then score the grout and remove the broken piece then take it to a tyle outlet to get the exact match of tyle then get the same grout color to place the tyle back you wont need fancy tools just tyle snippers or a $ 10 to $15 tyle cutter if it is in a corner but if its a hole piece then just apply grout and lay your tyle get a sponge and wipe clean its not very hard at all just explain at the tyle center that you dont do it for a living and you only need to change the one broken tyle

hammer and chisel it out. remove the grout carefully around it and remove the mortar below it then replace the tile with fresh mortar and regrout

You just need to break it up . Place a heavy towel over it and and break it with a hammer. Dont forget to use eye protection.
