In the US there are only a couple of medical school programs that accept high school students. UMKC (University of Missouri at Kansas City) is one that I know of. It is a 6 year plan and at the finish you are awarded an MD and a BS.
Most medical students in the US first went to a 4 year college, received an undergraduate degree and then applied to med school. No - You have to go to a 4 year college to get a Bachelors degree first.
Most students that want to go to MEd School go to a college that offers a Pre-Med program. THis program has courses that prepare you for Medical School
Medical School is four years as well. After which you become and MD and can practice General Medicine. Anything else requires still more schooling. Plan on being 30 before you come out and can practive medicine.

No, you go to a standard 4yr college. You must study hard and get good grades. NOT EVERYONE GETS INTO MED SCHOOL. They have so many applicants to med school that they can be picky. It isn't like going from high school to college. Everyone who graduates can get into a college (maybe not Yale, but "a" college). Not everyone who graduates from college can get into med school and continue on the road to becoming a doctor. My trainer at the gym wanted to be a doctor. However, his grades were not good enough. They weren't bad; they just weren't good enough for med school. It really does not matter too much where you go for your under-grad; just do well.

You go to a 4 year college and major in pre-med or a science that can be a basis for pre-med (i.e. bio pre-med or chem pre-med). Then you'll be able to apply to medical school and if you get accepted you will be in school for 4 more years.
