All depends on how they wear, you have to keep an eye on them. Believe it or not, I went 45,000 miles on goodyears on a 4x4 durango with NO rotation, they wore perfect. But that is an exception, once a year at least.

I never do, it is a gimmick that isnt needed. Your tire wears into a certain pattern on it's axel, move it to another axel and it has to wear back in causing more tire wear.

the simplest way to remember this is like this...
you change your oil every 3000 miles...therefore rotate your tires every 2nd oilchange..I find that it is easier to remember that way..

it depends on the tire quality, the vehicles weight, and the road condition.
every tires has margins...
when you already see a colored line or thread, replace it as soon as posibble.
