I know an Advance Directive is for health reasons if a person is unable to make their own decisions, but what is the name of the form to fill out saying to whom you wish to give your property, your money, your things when you die? What is the form called and where can I print one perhaps for free?

All that I'm aware of is that you can buy a kit to make a will at office supply stores. Papers will need to be witnessed and notarized--and not by anyone involved in the will. The stores should have only the forms that are legal where you live--be aware though, that all wills can be contested by family members, spouses, etc.

Its called a will an executor has to be appointed or an executrix, The person has to be of sound mind to be signing anything also because it could be threw out in court and the will could be contested.

It's simply called a Will. You can purchase a kit from most bookstores, including Amazon.com and Barnes & Noble.
