It refers to the severity of your episiotomy with your previous birth. 4th degree is the most severe.

The forth degree is referring to your previous tear or episiotomy during your last delivery. The 4th degree is the most severe and means that there was a laceration that extended all the way through your rectum and anal wall. I am sure that your doctor is concerned about that happening again and the amount of scar tissue in the area. I am guessing that he is weighing up the possibility of wanting to do a c-section instead. Good luck!

It means that you tore severely during delivery , 4th degree tearing usually means that you tore all the way through to your muscle and requires a lot of stitches.Your Dr. is probably concerned because it is so severe that you have a higher risk of infection and can lead to problems with incontinence. Some Dr's will recommend C-Sections if you have had previous experiences like this. Just talk to your Dr. and see what he says. The link below explains all of the different degrees of tearing. Congratulations and Good Luck!!
