Would really like to incorporate those existing items into the new 'robe, but like I said, the altered items don't fit snuggly in the waist (pockets in the way) and the slacks don't hang properly (too saggy in the seat and kind of limp starting at the knee). And the shoulders don't work at all!

Any suggestions would be appreciated.

Are you making the alterations yourself? ...or is someone else doing it? If you are doing it yourself books are available which show you how to alter your own clothes. Some books might also have instruction on reconstructing also.

In general clothes can only be altered no more than about 3 sizes smaller. I don't recommend reconstruction because that entails taking apart the clothing at the seam and sewing it back together at the new size which can be more expensive than the garment in question was when you first purchased it.

...Things to consider...
Anytime a seam is opened up (ripped) there are small holes left in the fabric. When your clothes are restructured and pockets sewn back on those little holes might be visible depending on the texture of the fabric. Anytime fabric is restitched near an area where it was stitched previously the seam will be weaker than it once was.
