It has to be like a play, with dialog and some direction for the actors, but with details of where the camera focuses and some details of the effects.

A bell rings:
Jack - (shoving all the magazines in the drawer) I'll be right there.
:: an explosion can be seen from a window in the frame behind Jack ::
Jack - (falling to the floor, moans) Ugh, the bastards.

I understand it has to be literate - for example, your question is ungrammatical; it should read, "What form should a movie script take?" (or something like that)
Here is a good website with directions and example of scripts:

Good luck.

It's often written more like a play. Place description really helps get across what you were thinking. The more you put into it, the more it will turn out how you envisioned. If your end vision isn't clear don't add so much and let the actors and directors bring their experience into it.

There are many websites online that allow you to read actual movie and television scripts, which should help you when drafting your own script. The Internet Movie Script Database [IMSDB.com] is a good place to start, as they have many new movies and television shows that would be interesting to look at!
