they can get very sick!! i was lucky my daughter did this just before she was born.
some babies need help to clear the lungs after they are born because it can cause them breathing trouble.
most of the time if the baby has gone inside it shows signs of distress which the doctors/hospital or midwife will notice during health checks ( hopefully)

First of all, its called meconium. Meconium is usually passed within 24 hrs after birth, however some babies pass it prior to birth. here is some further information on it:

All babies have meconium and will dispense of it through bowel movements at some point, hopefully after they are born. Meconium is the first bowel movements from a newborn that includes cells, mucus, bile, amniotic fluid and lanugo that is stored in the baby's intestines while in utero. It is dark green, almost black in color.

Sometimes however a baby will pass his first meconium while still in the womb, before the mother-to-be's water breaks. If this happens the amniotic fluid will be tinted green. In many cases of hospital births where the baby has passed meconium before birth, a special NICU team will be called when the baby is delivered in case there was ingestion of the meconium into the baby's lungs.

If a newborn does swallow some meconium it can lead to Meconium aspiration syndrome (MAS). While it is estimated that 5%-20% of newborns pass meconium in utero, only 5%-10% of those cases result in MAS. The newborn passing meconium prior to delivery occurs most often in postdate births. It can also be caused by fetal stress during the pregnancy or during labor and delivery.

If the meconium is ingested into the baby's lungs, three things can occur to the baby; however it does not always happen. Only a third of infants who ingest meconium need breathing assistance. The things that may occur include:

blockage of airways
lower gas exchange in lungs
inflamed airways leading to pneumonia

My son was overdue and had a bowl movement in utero. When my water broke it was a brownish color instead of clear. They just make sure everything is cleaned out. If they make one long before they are delivered, there may be infections.

You got several bad answers on this one, but duckygrl21 is absolutely right. My son and my neice were both overdue and both had a bowel movement before birth.

My niece had gotten some of the merconium in her mouth and nose, and they had to take her away and have it cleaned out asap. I'm not sure, but I believe they had to slide a tube into her stomach and pump some out. She ended up just fine.

My son was born by c-section because he was breech, and though he did pass merconium, he didn't have any on his head at all. I guess becuase of the way he was laying, all the yuck settled at the bottom where he feet where.

Then after he was born, he had several diapers full of merconium that daddy had to clean up because mommy was still all drugged up. =)

Everyone whos saying its not possible is WRONG.My nephew was overdue, and my sister had to have an emergency c section. he had a bowel movement and he came out like a green brown baby, Hes fine now, no further complications other than its kind of gross he was born in poo.

I guess I did that when my mom was in labor with me. The docs ended up inducing the labor to get it to go faster because they were worried that meant I was going try other post-birth functions - like breathing.

I guess it was a hard labor for my mom because being induced hurts more, but I came out just fine and healthy.

I believe the baby is nourished by the placenta, and all waste leaves through the placenta as well, ie. the bowels are not functional in utero.

My son did when i was in labor with him. So as soon as he was born they had a specialist be there to clean him out real good, just in case he inhaled any.

They just suction all the goop out of the baby's mouth.
