Is it 100% effective if taken properly? and when does it not work? ie wut medications might limit its effectiveness?
Also can i ejaculate safely inside if my gf is taking the pill properly?

have a read of the link below, there are also other considerations like the side effects on your girlfriend. it says that the pill is the most effective form of contraception other than sterilization.
remember that there are other things to consider, the pill won't protect you from sexually transmitted infections.
the link gives a list of medications that can limit it's effectiveness like antibiotics. it can also be effected by stomach upsets.

No it is not, my girlfriend was on the pill and taking it properly and she fell pregnant. its about 98% ish I think, it should say on the info sheet.
