My Manager has recently started asking me all these details (where i usually don't take much time off). We have a policy to take 3.5 hrs off from work incase of doctor appointment and I have enough sick time as well.

No, that's a violation of your privacy. Be diplomatic in answering by saying, "it's a personal matter and I don't wish to publicize it".

where i work (fed govt and have been a mgr) they can ask if they suspect sick leave abuse. they don't need to ask why but showing proof of the appointment might be reasonable. perhaps he suspects you have a chronic disease or are pregnant or something else that might affect the job. or that you are under stress or having problems with the job. i think it's reasonable for a manager to be concerned with a major change in behavior (use of sick leave). I would just show him the receipts or something from the appointments. but i agree that WHY you are going to the doctor is none of their business, only THAT you are going is their concern if it is not a workplace related injury or illness.

No, they can not ask you why you are going to a doctor. That is a personal matter and it is illegal.
