I'm not sure what you mean by "marks", but smoking's immediate effect is physical displacement of oxygen from the alveolus, which reduces the concentration gradient for diffusion. Chronically, it causes inflammatory reactions leading to destruction of alveolar walls, producing the condition known as emphysema, in which gas exchange is limited by expiratory obstruction.
