What are Hashfails and what can be done to make sure you don't get any and even repair hashfails?

How many hashfails needed to completly wreck the file? How serious are hashfails? and what can be done to prevent them?

Sounds as if it's some kind of parity check fault, but I would need more information. E.g. what program, Error message in full etc.
