You can either point this out to him or just start distancing yourself from him. That might make him realize his actions are bothering you and he'll change. That's hard to envision though if the person is really self absorbed.
Alot of friendships end. That's part of life & growing up. Just because you've known him for 13 years, that doesn't mean you have to be his friend forever. People change, sometimes for the worse and for your own wellbeing, you have to get them out of your life.

If he has become selfish only recently, then you have to find the reason for the change and rectify. If he has been selfish from the beginning and you have made the friendship, it is high time to leave him.

it doesn't matter you you knew first or who you knew the longest...friendship is about who came and never left...if your so called friend is being selfish...time to put your foot down and tell him what the problem is...its not his fault...maybe he doesn't realize what he's doing that may have hurt your feelings...so.......talk about it...its the best thing you can do at the moment...cause you know u cant just let 13 years go down the drain
sooo
all the best!

You don't have to leave your friend but certainly, you can make more friends, say a dozen of them. In four or five years, going by your sweet nature, half of them could become your fast friends and at that point, you can leave your present friend for good.

Try to assess yourself first, before you judge your friend. Ask question like who's making trouble with our relationship me or my friend? If there's something you think he is selfish, brought it up to him, but not the word "selfish" just tell him what you didn't like his actions for example. Best of all communicate to your friend.

The whole thing is, if you are truly his friend, you should say something. Your permitting him to treat you badly, what kind of friend is he to you? Kinda see what I am saying here? If you two have known each other that long, don't you feel you two could come to some kind of mutual respect? Believe me, you'd be doing your friend a favor!

Talk to him abt his behaviour, using words "I" and not "you", like if he is not keeping his appointment tell him "I felt unhapy not to see you that day, it could have been agood time"
Simply let him know how you feel bad in positive way. if it worls fine otherwise there is no ponin in keeping in touch with this person who dosent care abt u. though you can still be friends.
