You do not need to say anything! The phrase " Bless You" comes from times where the "Big Flue" killed a lot of people, and when somebody sneezed they blessed the person in the hope that they don't die of flue! It is a silly thing to say anyway in modern times but can be regarded as good manners and to respond with Thank You

Bless you origionated to combat the plague.
When someone sneezed (seen as the initial onset of the plague), they were immediately blessed ("God bless you!") in the hope that they would not actually develop the disease.
