The whole movie was disturbing, from the sex scene with the son sexually assaulting the girl and a chicken at the same time; the man doing the "Anus Dance" at the wedding of the retarded old lady and the "Egg Man;" to Divine, a transvestite looking like "Killer Clowns From Outer Space," picking up and eating dog poop.
Actually, Divine is always disturbing...

Surprisingly, it's nothing gory. How the film ends is the main factor so... the ending of the first Saw. It ruined it for me. I could have handled it a lot better if the ending was different.
But the least disturbing scene in a horror film is the feet breaking scene in Misery. It was really funny because Kathy Bates' delivery was so deadpan.

The part where they are skinning the person inTexas Chainsaw Masacre the Beginning... Or where they are pulling those rings out of that persons skin in Saw 3...
Edit: Now that I think about it, those two movies are disturbing to me now, but nothing can really compare to how freaked out I got when I watched Pet Semetery when I was what, 6? I wouldn't sleep on top of a bed for months because I was scared of that kid with the scalpel...

In the French movie, Cache, there is a suicide scene that was so unexpected and the arterial spray so convincing to this nurse who has seen about everything,that I cursed out loud and felt hot pins and needles all over for several seconds. Its a good thing I was watching at home because my startled reaction was LOUD.

Actually, i just changed my mind... the rape / gore fest in The Hills Have Eyes. I honestly think that was the single most disturbing scene in a movie..... & I dont really get 'disturbed' that easy.

Actually, it's a scene I DIDN'T see because I couldn't stand to watch. In the original Hitcher movie, when Jennifer Jason Leigh is torn apart between two semis.

when the penguin gets beaten and dies in the movie Five Corners with Jodie Foster...I really don't mind if people get killed in movies, but when they start killing animals, it pisses me off. The poor little penguin. Just like at the end of Wicker Man (at least the classic version- I'm not going to see the crappy new one) they stuck all those animals in the wicker man at the end and burned the thing, and that ruined the whole movie for me.

the whole movie "Irreversible" was disturbing. It is banned in the USA you can get it online though. There is a gruesome fire extinguser scene and a horrid rape scene. Awful...
