If you are off work on disability you will need to get a written doctors note releasing you to return to work. Your current employer should not let you return until they receive this note.

should go back 1st working day of the 30th or go back and get another sick note from 30th till whenever, it's just as he says 7 days from and including the 24th, if you feel no better see him/her on the 30th and get another one to carry it on, simple really, make sure you get your sick pay!

How do you feel? If you have not improved go back to your doctor and get an extension. If you feel capable of working, then you return to work tomorrow.
