I still take off my hat in elevators. I open the car door, pull out her seat for her...etc.. I'm with you all the way man. Treat a lady the way she SHOULD be treated.
Edit:
Why would women not like to be treated with respect? What's the deal "trying to be equal". Fine, lets all go treat women like dirt. No? Then accept the fact that a guy is trying to be courteous towards you. It shows that he respects you. Get a grip.

Sorry......but I think you'll find most guys do not remove their hats for a lady in the elevator. From what I see most guys don't take off their hats when sitting a the dinner table.
As for walking on the street side of the sidewalk....yes, I still do that and I hold doors open for both men & women.

I always do my best to treat ladies with respect like you mentioned. I usually do not wear a hat indoors so the same goes for an elevator. When entering someones house the hat comes off, bad hair or not. I stand when a lady gets up to excuse herself from the table. Open or hold the door for ladies. Open the car door for any ladies getting into or out of my car.
I think it's pretty sad that so many people find it an inconvenience to be courteous to each other.
(Added) I also think it's pretty sad that someone uses the excuse that woman do not want to be treated as a lady.

I never heard of taking off a hat in an elevator. But I have heard about walking on the street side. I used to date someone who said to me "You're not up for sale, get on the inside of the side walk"

i've never heard of either of those "old-fashioned" customs... remove your hat indoors or at the table, sure, but an elevator?
chivalry is a tricky thing. some women, myself included, are insulted by being treated differently. i would think some men would be offended by being expected to do extra things like pay for dinner, open the door, etc. just because they are male. i would. we should practice common curtosy towards one another regardless of gender.

I never do it because girls don't like it. Same with standing when a girl walks in the room. Can't open the car door for a girl because they are already out before I get my own seatbelt off. Can't pull a seat out for them because they are already sitting down and eating before I get a chance too. Todays women are not as lady-like as they were in the past and lack the class that they used to have. Feminism has shown that women want to be equals with men, so they dont want to be treated unequally by having their bf pay for everything or act like cary grant from the 1930s.

I don't wear a hat very often but I take it off when sitting down to a meal. I don't remember ever wearing a hat on an elevator.

I agree that men have lost much of there mannerisms with woman. Most of that has to do with woman lib. Woman wanted the same place as men and they got it. If men have to compete with woman in the workplace all day it is pretty hard to see them as needing your protection.
Amoung Practiceing Muslims most men still behave with the best of manners and work very hard to protect and treat the woman in there lives with respect. My faith says that the best of men are the best to their wives.
I also belive this is probably true of most religious men.
