Make a list of the private colleges which offer admission on 'donation' basis: select the states of your choice: narrow down to three states: then, do a final screening, and select five colleges for further scouting. Go to the colleges, with parents or elderly friends who may adise you: find the fee structure, the details of their admission procedure, including tests, documentation, date lines etc. Make notes carefully and systematically. Proceed to select the two or three colleges for a final list, and then go ahead and register your application! Good luck! (does not matter if you are not in the merit list, but you can still become one of the best doctors in the world, in your time!)
