how is it possible to have people right click an image and they think they saved that image but when they look at that file, the image is completely different?

It has something to do with a hidden .gif or an image.
a website or code would be much appreciated.

If the site doesn't want you to download usually copyrighted images then they protect them by redirecting to another image.

One way is to take a screen shot then edit the picture from it.

If it is set on the web page then there is very little you can do.
Try contacting the site owner for a copy.

Well that is called stenography, or the process of hiding an image in another one.
