DO NOT INCLUDE YOUR CURRENT SALARY. Wait until they ask you how much you want, or they make an offer. Then you start negotiating with them. Ask for more than what you are willing to accept, then let them talk you down. Include what you want for vacation time as well.

I would Not include my current salary on my resume. That could hurt you getting the new job. I would wait for the interview to see what money offering your boss will be willing to pay you. Most of the time it is negotiable.

no not your current salary. if they ask for your current salary give them a general idea
45,000 - 50,000
or you can say and i have seen people type that they are looking in a certain range like 50,000 - 55,000
if you really want a particular job, wait until you get there to discuss salary range otherwise you might scare jobs away

Unless the potential employer specifically requests it, it is more likely to hurt you than help. If you are currently making more than they are considering offering, you will be ruled out immediately. If they are planning on offering much more than you currently make, they may lower their offer to you.
