When Samuel Johnson wrote the first English dictionary, he left it out. Nobody knew what a dictionary was back then, and the book was marketed as "a compilation of definitions in the English Language," the cover simply read "English dictionary." Of course, everyone who bought the first edition immediately looked up "dictionary" and, when it wasn't there, well, you should have hear the Tsking and Pshawing that went on! Samuel didn't make that mistake again, and nobody else has either.
