You can find the forms to apply for grants at Grants.gov http://www.grants.gov
You will be required to register first as an individual or as an organization (if you are representing one).
Then you have to find a grant where you qualify. You can find the list of grants at the Catalog of Federal Domestic Assistance http://www.cfda.gov - go over CFDA grant listings and take note of the CFDA number of the grant. You will need that number to download the correct application form from Grants.gov
