Whats the movie with Amy Jo Johnson where she plays a dead teenage girl?

From what i remember she died by falling off a bridge in a car with her boyfriend in it. Then some high school teen somehow can see her and he helps her, his the only one that can see her. at the end she gets into the car with her boyfriend and some old guy ( i think her grandpa) and they go into this cloud?
