I have a page on my website that has some unique activities for baby showers....unique SENTIMENTAL activities. It's refreshing to have some sentiment mixed in with the silliness. There are also some great favor ideas below the games!!
Also, I make diaper cakes....which make for a wonderfully unique centerpiece at the shower. Conversation starters, for sure....and really memorable. PLUS....it can double as your sister's gift to you. Everything in the cake is usable by the baby (or you!)

I attended a baby shower over the weekend and it was fish themed, I guess they themed it after what the baby nursery is going to look like. She set out a platter of food to make finger sandwiches and she also kabob fruit, which I thought was cool. We played three games, a word scramble game that were baby words like cradle and baby swing jumbled up and we had to figure them out, who ever got the most words won, then we played a game where everyone cut a string that we thought would be the size of the mothers tummy and then they measured the mom after everyone was done cutting their string and who ever was the closest won a prize.... the last game we played, we all got a sheet of paper that was labeled a-z and we had to go down the sheet trying to name as many baby items as we could with each letter, ex: a-apple sauce, b-blanket.... who ever had the most won....

Baby Shower Games:
1.Bubblegum Baby
Give each guest a piece of gum and have them chew it but don't tell them what it's for. After about 5 minutes you tell them to take out their gum and form a baby with it. After they are done creating their baby, place it on a paper plate and wirte the name of the creator of the gum baby. The mommy then picks the one she likes the best.
2.Who's That Beautiful Baby?
When mailing the invitations, enclose a request to have each guest bring their baby picture to the party. At the beginning of the party, you slip into another room and put each photo into a ziplock baggie with a number showing on the back. Bring out the photos on a bulletin board tacked up using the edges of the baggie and have each guest guess who belongs to which picture. It's so much fun! The one who guesses the most wins.
3.Don't Cross Your Legs!
Upon arrival give your guests a clothespin to pin to themselves. Throughout the course of the shower anyone who sees someone crossing thier legs/feet gets to take that person's clothespin. At the end of the shower the person who has the most clothespins wins!
4.Poopy Diapers!
You really have to have a sense of humor on this one. Take 3 to 5 candy bars and place one in each cloth diaper. Microwave each diaper for 30 seconds - or until the candy bar has melted. Have each guest take turns sniffing the diapers and guess what candy bar is inside. It's hilarious watching everyone sniff -- makes for an interesting game.
5.Guess Mommy's Waist Size
Have each guest cut off a piece of string that they think would match the size of mommy's tummy. After every guest has a piece of string the length they desire, have each take their turn placing it around the tummy of mommy. This can be hilarious as some guests' strings will be far too long and others wouldn't make it around the corner! The guest who comes closest to the tummy size wins. Rule: No guests can put the string around themselves or each other to make a guess.
6.My Water Broke!
A day or two ahead of the party, place in the freezer small or medium sized plastic babies inside plastic cups filled with water. Have one cup for each guest. In the beginning of the party, give everyone a cup with their baby frozen in the ice. Guests are not allowed to touch the cube with their fingers, but are allowed to hold the glass. At some point during the party, the ice will melt and their baby will become freed - thus, their "water will have broke"... The first one whose water breaks is the winner.
7.Don't say "Baby"
When the guests arrive, give each a clothespin to wear. Tell each guest that they can't say the word "BABY". If a guest catches someone saying "BABY", they may take the clothespin away from them. If, for example, a guest has 3 pins from previously taking them away from others and somone catches her saying "BABY" then that guest now gets to take all 3 of her pins. The person with the most clothespins at the end of the shower wins the game.

i went to one where everyone got a bottle of shower gel that had been wrapped in white cloth and a rubber bottle nipple hot glued to the top of it. the card said "From our shower to yours" Super cute.
We also played baby bingo - just like regular bingo but with baby related items instead of numbers. look online or at party stores for the cards.
also, she had 40 people there, and opened all the gifts one by one. i know it might be tradition, but yaaaaawn. after the 10th gift I just didn't want to see her having to act suprised and thrilled at the next jumper or baby sign language book. so maybe open it up to your guests - if they really want to see you open their gift, have a chunk of time set aside for that, but the rest of the guests can continue on with the party.
keep it simple. with good food and music and a game or two the ladies will be totally entertained. It's mostly about seeing you pregnant and happy anyways!
