He'll be called by his name. (ie. Mr. John Doe), just as the first Lady is now Laura Bush. I don't think he'd be considered the first gentleman. I think the first spouse would be a little easier on the tongue than the first gentleman.
In Europe the king's wife is the queen, but the queen's husband is not the king, he's just refered to by his name (in case of royalty that includes his title or just plain "His Excellency")
