There were other empires - Assryain, Sumerian etc but these tended to rise and fall fairly quickly. There were many city states on the Italian peninsular at the time of the rise of Rome - many founded by the Greeks - known as Magna Graecia. Rome always regarded the rest of Italy as subordinate, although fairly early on Roman citizenship was extended to all the inhabitants of the area.

Because Italy had many different cultures in it at the time of Rome's rise. Rome did not even conquer all of Italy for sometime after it had become a major power. Also, the Romans called themselves Romans and not Italians.

The city of Rome was the greatest and brightest city in the Roman Empire, with the possible of exception of Constantinople, after its been founded by the Constantin the Great. This was surely one of the reasons why did they call themselves Roman Empire. Also, the Empire's heart was the Rome, so it is proper to call it Roman Empire.

There was no Italy in the beginning of the Roman Empire. There was just the Roman Empire. Think of it like, there was no individial states before there was a United States. The country is still here, but there wasnt states.Italy came after the Roman Empire and was partof it.
